Referral Process



Our Referral Process

We aim to make referring to Distinct Directions as simple, transparent, and supportive as possible — for clients, families, and referrers alike. Below is an overview of what to expect once a referral is made.


1. Submit a referral

A referral can be made by a client, family member, Support Coordinator, or other professional by completing our online referral form.

Sharing as much information as you can helps our team better understand what support is needed.

If you would like help to complete the referral form, please contact our friendly admin team who will be more than happy to assist you over the phone or in-person in our office.

2. We review and get in touch

Our Intake Team will carefully review the referral and reach out if any extra details are needed.

They will also talk through current availability, likely timeframes, and next steps, and answer any questions you may have. This may be via email or phone call.

3. Matching you with the right clinician

We take care to match each client with a clinician who is well-suited to their needs, goals, and preferences.

When a suitable clinician becomes available, we’ll prepare a Service Agreement and send it to the client’s decision-maker or Support Coordinator (if requested). We may contact you to confirm funding details, such as NDIS Plan Dates, as these can change.

4. Service Agreement signing

The Service Agreement is valid for 14 days. Once it is signed and returned within this timeframe, we can move forward with booking appointments.

If you are not able to complete the Service Agreement signing in this period, please contact us and let us know so that we can work together to navigate any issues.

5. Booking your first appointment

After we receive the signed Service Agreement, our Intake Team will contact you to arrange your first appointment.

A quick note about “fit”

Feeling comfortable with your clinician is important. If at any time you feel your clinician isn’t the right match for you, please let us know.
Where possible, we’ll work with you to explore other options and support a better fit.
